Q: Are they fully plastic?

No the orange part is plastic and the black part is rubber

Q: How long screw do i need and whats the best spacer size?

These are smaller than the traditional offroad wheels so I used 2" mounting hardware with 1" risers ( two 1/2" risers stacked).

Q: Come With or Do these need bearings?

Unfortunately they need bearings, but the wheels are awesome
